Your Medical Records

We need to keep notes on your condition, treatment and progress to give you the best possible care. All staff who have contact with your records are obliged to maintain confidentiality at all times and to keep them securely.

Access to Health Records

You have the right to have a copy of your medical records, subject to a charge. To access them, you need to apply using the Application for Access to Health Records form and return it to the Access to Health Records clerk in our medical records department.
